While our goal is always to aim for 100% accuracy to allow straight-through processing, there may be instances that require human QA, such as a distorted source image. Fortunately, the tools we use can determine confidence levels to set thresholds for when something should go to QA. The QA interface is user friendly, allowing a user to correct what the software is unsure about efficiently.
